DAILY PAIN CHART DIARY

    I know alot of us have been looking for sometype of a Daily Diary to chart our pain.

    I know this is going to be a very big help for me to prepare for my Dr. visits.

    Let me know what you think and if there are things on this chart that you would change to make it even better.

    I was wondering if I should bring the chart to my Dr.?

    Or do you think that may be going a little overboard

    I really like my Dr. and I dont want him to think I am a difficult patient by showing him the chart.

    What do you think. Anyone ever chart thier pain and bring it with them to show the Dr. And if you did show the Dr. it, did the Dr. find it useful?

    I go to a pain clinic so I can get my Oxycontin. You have to sign an agreement with the Dr. and if you ever break your part of the contract, the Dr. will wein you down off of your narcotics and discharge you. There are several things I had to agree to do. Some of them are:

    1. Keeping a pain diary and presenting it when asked.
    2. Do not self medicate or deviate from the prescribed
    dose.
    3. Having all prescriptions filled at the same pharmacy
    4. Submitting to a urine test when asked.
    5. Keep all monthly appointments (if you don't go to your
    appointment-no meds).
    6. Bring in the bottles of all prescribed narcotics and
    etc.
    7. You have to have a safe to lock up all ofyour narcotics
    8. If you lose your meds or they are stolen you have to
    file a police report and that still doesn' gurantee
    you'll get a replacement prescription.

    I'm sure there are some docs who don't want a pain diary but mine finds it very helpful when prescribing my mdeds. LuAnn

    Another thing the diary might be useful for is if your applying for SSI, or if your on FMLA or Disability at work.

    Has anyone used a daily pain diary?

    If so, did your Dr. appreciate it and find it useful?
