Disability Letter from my Doctor

    Great news, my doctor is willing to write me a letter regarding my disability. Her request was for me to give her the following:

    Hello,
    Pls send me a note detailing what of your customary job you aren't able to do--subjective restrictions regarding lifting, sitting, walking, standing, typing, fine hand coordination/manipulation.

    My question is what does "subjective restrictions" mean?? Has anyone heard of this?

    Also make mention of how much the side effects of your medications effect you. Also on any form that they give to you to fill out always put the details of your worst days that are filled with pain.
