I realized on my last visit that each time I see my doctor I fill out the pain questionare, my answers are going to be different, depending on which room I am in.

In the waiting room, the chairs are very uncomfortable, and they make my legs and lower back hurt.

In the exam room, the exam table makes my upper and lower back hurt, along with my arms and legs.

Does anyone else have to fill these out, and what is the point?

I have only done that at a pain clinic once. I think those are meant for what your pain has been like since your last visit not at the exact moment that you're there. If it says "what's your pain like today", I'd think they mean in general? or that's the way I'd answer, actually I'd tell them the worst.
I'd ask your Dr. what it means to them.

Its very frustraiting to answer the question of "what hurts the most" at that point and time.

I did try to explain to the doctor that they are going to get a different answer, depending on where I am at, when they ask me that. Especially when I am sitting on that very uncomfortable exam table.
