Fibromyalgia Questionnaire for Dr. Visit - Maybe you can use it..

    Hello Guys -

    I was doing some searching on the Net, looking for
    a local doctor . . . and in the process found a
    web site that had a great questionnaire that I
    thought may prove to be very helpful when going to
    see a doc for an initial or follow up appointment.

    I know that I had a similar questionnaire, though not
    as thorough, when I 1st went to a Rheumatologist
    and he found it to be
    very helpful in diagnosing me, saying it was a time
    saver too. I know that for me, I get really really
    tired, even ashamed in a way when I must state all of
    my symptoms when seeing a new doc. It's like it makes
    me feel worse just having to REMEMBER and then list the
    long symptoms.

    to the link listed below.

    There's a shorter one there too, called the Fibromyalgia
    Impact Questionnaire. This one is used alot in research studies to get a pre and post numbers.

    Thank you for the link. I understand how tough it is to explain myself AGAIN every time I see a new doctor. I have avoided medical appointments because of this, that I just can't face it again.

    Thanks for responding.

    My thought is that it is just too much work to
    go to yet another doctor and having to put ourselves
    out there, ie: be vulnerable, hoping that the doc
    will be able to really help us and not say something
    to trivialize our suffering.

    The other issue is that because we may have 3 or
    more issues to discuss, the doc will usually cut
    us off after the first or second concern because
    we're "out of time", so perhaps this questionnaire
    or the other one I mentioned, will allow the doctor
    to see the overview before we even start.

    I know the doctor I had gone to was appreciative
    of me bringing in a comprehensive questionnaire
    already filled out. He said that he may continue
    using it with other patients.

    It's a good one.

    Looks like it may have been done by a University Clinic.

    Hello -

    I wouldn't want anyone to miss this one.

    It is especially good when you go to a new doc
    or want to update an old doc with your most
    prevalent symptoms.

    I put it on my favorites and will download and fill it out before my next dr. appt.

    I may even look for another doctor. Mine just doesn't seem too impressed with FM...he believes it makes one hurt, but he thinks I should just be able to ignore it because it really isn't a pain that is doing damage to my body...

    NO, just my whole life! Sheesh!

    He was good for other things but I think it's time to look around.

    Anyway, thanks for the helps me a lot.

    I know this is an old post, but I am probably going to have to see my doc and have a real heart-to-heart with him about the pain and FM.

    I may even have to go find another doctor, actually.

    So, as I liked this survey, I have printed it out and hope this will bump this up and let others see it again.

    Dear Jillian,

    What an awesome questionnaire! It's so wonderfully thorough and detailed. Perhaps you should post it on the chit chat board too?

    If docs will read this questionnaire over thoroughly it can hopefully accomplish many things:

    1. They will understand what we are feeling and experiencing from day to day and week to week.

    2. They will know what questions to ask us in our doctor's visits.

    3. Perhaps they might begin to think about how to better treat us. (Wouldn't that be fantastic?! A person can dream, eh?)

    We need to keep this bumped indeed. Thank you!
