intermittent painful lumps on top of head

I have several AI chronic illnesses and I'm trying to figure out which one may be causing this present condition. I haven't noticed a pattern with these sore lumps, they seem to come and go at will. They get so sore it makes it nearly impossible to brush my hair, wash my hair, or even touch the top of my head without causing much pain. I know it may sound strange, but if I grab a clump of hair and just tug at it slightly for a second, that gesture eases the pain for a moment. I do have headaches and imbalance problems at times, but have no idea if there is any correlation. The last doctor ( my primary) suggested it was stress. Somehow, that doesn't ring true for me. Thank you for any suggestions.
