If I can recall well enough,.....
There was only a 1 page questionaire.
    Maybe 5-6 questions.
One being do you think your condition has improved,..stayed to same or gotten worse since you we're approved.
have you worked or thought of returning to work.(duh!!)
and a space to write down drs.appointments you've had in the last yr,any tests,or anything medical you've had done recently.
and drs. names and phone #.
    Thats about it.
    It was'nt that bad.
Nothing compared to all the questions when your trying to get approved.!!!
    Good Luck to you too!
    Hope this helps :)

Hi S.S. asked questions like have you done any work since you've been on ssi? List all doctors, hospitals, and medications you have taken since you started ssi, and do you feel that you are is better or worse? I think I filled out about 12 pages on my conditions. Thanks, Virginia

I also had to go to another of their doctors - I thought that was normal but I guess not. I did a bunch of forms too & sent in letters from people who knew me. AMY

keeping drs. appointments, and records/dates are very important.
I see a dr. for my FM and back problems about every 2-3 months. and My gyno for Endo every 10 weeks.
    and then tests inbetween sometimes.

    I keep track of everything~
    Hope this helps :)

I never had to see one of their drs. for a review.
(only in the very beginning when I was trying to get approved) I've had 2 reviews so far.
