Which of your doctors usually fills out your FMLA forms?

    I am going to try a new Family Practice doctor tomorrow, who is Fibro Friendly. She sent a nice packet on Fibromyalgia and even wrote a personalized note to me stating she looks forward to meeting me. She is a new, very young doctor. I'm wondering if she will complete my FMLA forms or if she'll refer me back to the Osteopath who diagnosed me. I currently have FMLA, but I'm tired of running around to get the form updated every few months. I just want a doctor to say that my condition is indefinite and just have to worry about a recertification once a year.

    since he's known me for over 16 years. He's the one I go to for everything, unless I need a referral to a specialist.

    I think for FMLA, they pretty much are expecting your regular doc to fill out those forms, unless you've just had surgery or some other special procedure done. Hope that helps! PG

    Still have to do the forms every 3 months .I did it for a year and than the day came were I was terminated due to illness.

    You might want to ask your new doctor. Since she is Fibromyalgia-friendly and will be your doctor of choice, she can follow your case better.

    When i got my latest doctor (with him three and a half days now) I got a jury duty summons and sent all of the papers to him. He filled them out and got me out of the jury duty.

    Reason: the disease, no car and i would have to walk to and from a bus, which I would have to take.

    So, it might be good to establish a really good relationship with the new doc.
